Checklist for College Applications
All of the
following steps must be completed before your college application(s) will be accepted for processing.
1.
A FERPA
(Family Education Rights & Privacy Act form linked above) must be
signed and dated by a parent/guardian and returned with your
application(s) before we can send transcripts.
2.
Your student activity Sheet must be on file in the
Counseling Office. This form is provided by the Counseling
Office. The Counseling Office will not accept your application
without this.
3.
Pick up Woodhaven High School envelopes for the
schools you are applying to if ACT scores and transcripts need to
be sent from the Counseling Office. Address the envelope to the
appropriate College Admissions Office. You will need
one stamp if
you are applying online and two stamps if you are applying with the
paper application.
If you are applying online:
1. Fill out the application completely and pay
online or send a check/money order for the application fee.
2. Print out the counselor page before submitting
the application and bring it to the counseling office with the
addressed, stamped envelope so we can include your ACT scores.
3. Go to
www.docufide.com to create a free profile so you can request
that your transcripts be sent to the college/school you are applying to.
More specific directions are in the counseling office.
If you are applying with a paper application:
1. Fill out application completely.
2. Bring in the application with a check/money
order for the
application fee with the addressed, stamped envelope so we can
include your ACT scores.
3. Go to
www.docufide.com to create a free profile so you can request
that your transcripts be sent to the school you are applying to.
More specific directions are in the counseling office.
Note:
Colleges do not request transcripts and ACT
scores from WHS. You
MUST request them from your
counselor. If you are applying to Western, U
of M, or Michigan
State, you must have your ACT scores sent directly from ACT.
Login to
www.actstudent.org to do this. If you
already had your scores sent to
the college you are applying to when you took the ACT, then we
will not need to send your scores through the mail, however, you
will still need to create an account with docufide to request that
your transcripts be sent.
BRING COMPLETED APPLICATIONS, ENVELOPES, AND FORMS
TO THE COUNSELING OFFICE.
Don’t forget to sign the application log book
located on the secretary's desk in the counseling office.

Please Note:
The 40 hours of community service is no longer a WHS graduation
requirement and is no longer a requirement to be considered for the
MME Merit Scholarship. However,
eligibility for MME Merit Scholarships is
still based on whether a student exceeds (score of 1) or meets (score
of 2) state standards. Taking the MME
remains a WHS graduation requirement. |
